A standard, generally well-tolerated combination for Wilms tumor. The main things to watch are nerve effects and constipation from vincristine (use the recommended bowel routine), and lowered blood counts and mouth sores from dactinomycin. Uncommonly, dactinomycin can affect the liver — the team monitors bloodwork.
Get help straight away — this is true on any regimen, on any day.
A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.
Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

These build with the number of doses, or are measured from when treatment started, so a cycle day would be the wrong shape for them. Each keeps the timing its own source states.

Vincristine can cause numbness, tingling, or weakness (including foot drop or jaw pain) and commonly slows the bowel, causing constipation — occasionally a more serious blockage. Use the recommended bowel routine from the start and report constipation, belly pain, or new weakness.
Call your team: No bowel movement for 3 days with belly pain, or new weakness/trouble walking.
Dactinomycin can lower infection-fighting white cells, platelets, and red cells; counts are checked before each dose. Watch for fever, bruising, or bleeding.
Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.
Sickness in the first 24 hours is the "acute" phase and usually peaks 2-3 hours in; anything from 24 to 120 hours is the "delayed" phase.
Usually mild-moderate and controlled with anti-nausea medicine.

Uncommonly, dactinomycin can cause a liver problem (veno-occlusive disease) — signs include belly swelling, tenderness over the liver, weight gain, and yellowing. It's more likely in young children. The team monitors liver tests; report these symptoms.
Call your team urgently: Belly swelling with pain, rapid weight gain, or yellowing of skin/eyes.
Hair loss is common and regrows after treatment. Dactinomycin can also 'recall' a skin reaction (redness/soreness) in an area that was previously irradiated.
