Imatinib (Gleevec): what to expect

CML (Chronic Myeloid Leukemia)Taken continuously — no infusion cycle8 effects documented

A daily targeted pill for CML that most people take long-term with good tolerance. Its most recognizable effects are mild fluid retention (often puffy eyes/face) and muscle cramps, plus some nausea, diarrhea, and rash. Blood counts are checked regularly, especially early on.

Oral imatinib taken every day, continuously (a targeted BCR-ABL inhibitor). Most people tolerate it well long-term. Its hallmark is fluid retention — especially puffiness around the eyes and face — along with muscle cramps. Onset is measured in weeks-months, not infusion cycles.

Get help straight away — this is true on any regimen, on any day.

A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.

Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

What is reported

This regimen is taken continuously, so effects are described in the terms their sources use rather than by cycle day.

Fluid retention / puffiness (edema)

first weeks-months; dose-related

Imatinib's hallmark effect: mild fluid retention, classically puffiness around the eyes and face, sometimes ankle swelling. It's usually manageable (reduce salt, sometimes a water pill). Rarely, more serious fluid buildup (in the lungs or abdomen) occurs — report rapid weight gain or breathlessness.

Call your team: Rapid weight gain, marked swelling, or new breathlessness.

Muscle cramps

early; can persist

Cramps (often in the calves or hands) are a common, characteristic imatinib effect. Staying hydrated, stretching, and sometimes electrolyte/calcium measures help; tell your team if frequent.

Nausea

early weeks

Common and usually mild — take the pill with food and a large glass of water. Anti-nausea medicine helps if needed.

Muscle and joint pain

Aching muscles or joints, usually manageable with simple measures; tell your team if it limits activity.

Diarrhea

Loose stools, usually mild. Hydration and anti-diarrheal medicine as advised help.

reported by about 45% of people · from Imatinib

Rash

A skin rash, usually mild and manageable; tell your team if it's widespread or bothersome.

Fatigue

Tiredness that usually eases over time. Pace activities and stay active as you're able.

reported by about 39% of people · from Imatinib

Low blood counts

first 1-3 months; usually recovers

Drops in white cells or platelets, most likely in the first few months, then usually settling. Your team checks bloodwork (weekly at first, then periodically).

Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.
