A three-drug regimen for stomach/esophageal cancer. Watch for low blood counts and strong nausea; the heart function is monitored because epirubicin's risk builds with total dose.

Epirubicin + cisplatin + continuous-infusion 5-FU, every 3 weeks. Epirubicin adds a cumulative, dose-limited heart risk; cisplatin adds kidney/hearing effects and strong nausea.

Get help straight away — this is true on any regimen, on any day.

A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.

Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

Timed to the cycle

Neutropenia

Typically around days 9–39

A temporary drop in infection-fighting white cells — the most common serious effect.

Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.

Not tied to a cycle day

These build with the number of doses, or are measured from when treatment started, so a cycle day would be the wrong shape for them. Each keeps the timing its own source states.

Heart-muscle weakening (cardiotoxicity)

Epirubicin (an anthracycline) can weaken the heart's pumping over time; the risk rises with total lifetime dose. Your team tracks heart function (e.g., echocardiogram) and caps the cumulative dose.

Call your team: New shortness of breath, swelling, or a rapid/irregular heartbeat.

Nausea and vomiting

Sickness in the first 24 hours is the "acute" phase and usually peaks 2-3 hours in; anything from 24 to 120 hours is the "delayed" phase.

Strongly nausea-causing (cisplatin); you'll get robust anti-nausea prevention, including for delayed nausea over the following days.

Call your team: Can't keep fluids down for more than 12 hours.

Kidney injury (nephrotoxicity)

Cisplatin can stress the kidneys, so you'll get IV fluids around treatment; bloodwork is checked each cycle.
