Doce-Gem: what to expect, day by day

Sarcoma - Soft Tissue21-day cycle6 effects documented

A two-drug infusion pair for soft-tissue sarcoma. Expect lowered blood counts and tiredness; docetaxel can cause gradual nerve tingling and fluid retention (you'll take a steroid to reduce swelling), and gemcitabine may cause a brief flu-like feeling after treatment.

Gemcitabine + docetaxel every 3 weeks (often for leiomyosarcoma). Blood-count drops are nadir-based (~day 8). Docetaxel adds cumulative nerve tingling and fluid retention; a steroid is given to reduce the fluid retention. Gemcitabine can cause a transient flu-like feeling after the infusion.

Get help straight away — this is true on any regimen, on any day.

A temperature of 100.4°F (38°C) or higher during treatment can mean a serious infection while your blood counts are low. Call your team or go to the ER — do not wait it out.

Also urgent: trouble breathing or chest pain · swelling of the face, lips or throat · new confusion or fainting · shaking chills · not being able to keep fluids down.

This is standing guidance shown to everyone. Nothing on this page is based on anything about you, and nobody is monitoring you.

One of the 6 entries below is marked provisional. They are shown because leaving them out would be its own distortion, but they have not yet been checked against the source document by a person, and they are not presented as authoritative. Every provisional entry is labelled where it appears.

Timed to the cycle

Day 1 is the day of your infusion. These are the effects a retrievable document ties to a day range for this regimen — not a prediction about you.

Flu-like syndrome (gemcitabine)

During or just after an infusion

Fever, chills, and aches within a day of the gemcitabine infusion that usually pass quickly; acetaminophen helps. Tell your team so infection can be ruled out.

reported by about 41 (fever); 19 (flu syndrome)% of people · Timing is part of what this effect is · from Gemcitabine

NeutropeniaPROVISIONAL

Typically around days 7–17

A temporary drop in infection-fighting white cells; the low point is usually around day 8. A white-cell booster is often used with this combination.

Call your team urgently: A temperature of 100.4°F (38.0°C) or higher — an emergency.

reported by about very common% of people; severe in ~25-40% · Timing from the manufacturer’s prescribing information · from Docetaxel, Gemcitabine

Low platelets

Typically around days 7–17

A drop in platelets (which help blood clot). Watch for easy bruising or bleeding.

Call your team: Bleeding that won't stop, or black/tarry stools.

reported by about 24% of people · Timing from the manufacturer’s prescribing information · from Gemcitabine

Not tied to a cycle day

These build with the number of doses, or are measured from when treatment started, so a cycle day would be the wrong shape for them. Each keeps the timing its own source states.

Fluid retention / swelling (edema)

builds over cycles

Docetaxel can cause fluid retention and swelling that builds over cycles. You'll take a steroid around each dose to reduce it; report rapid swelling, weight gain, or breathlessness.

reported by about 60% of people; severe in 9% · from Docetaxel

Peripheral neuropathy (cumulative)

builds over cycles

Numbness or tingling in the hands and feet that builds with repeated doses; report it early so the dose can be adjusted. It often improves after treatment but may persist.

Call your team: Numbness that makes it hard to button clothes, walk, or feel the floor.

reported by about 58% of people; severe in 6% · from Docetaxel

Fatigue

Tiredness that builds through the cycles. Pace activities and rest.

reported by about 66% of people; severe in 15% · from Docetaxel, Gemcitabine

Timing windows on this page rest on:

A window appears here only where a retrievable document ties it to a drug in this regimen. Where nothing could be found, the window was removed rather than estimated — so a missing time is an honest gap, not an oversight.
